Implementing the U.S. Agency for Healthcare Research and Quality’s Hospital 2.0 Survey on Patient Safety Culture

This project used the Agency for Healthcare Research and Quality Hospital Survey on Patient Safety Culture 2.0 to assess the current patient safety culture (PSC) and attitudes among Certified Registered Nurse Anesthetists (CRNAs) and physician anesthesiologists at two acute care Southern California facilities. Additional aims included assessing demographic data, comparing responses by provider type across survey dimensions, and providing recommendations for improvement. 121 surveys were electronically distributed and yielded a sample of 43 anesthesia practitioners (36% response rate) for this cross-sectional study design. 49% of survey participants responded positively to the overall “patient safety rating” of the department, demonstrating room for improvement. The dimensions (composite measures) with the highest positive responses were "teamwork" (71%) and "communication about error" (65%). In contrast, the lowest positive responses were seen in the dimensions of "staffing and workplace" (21%) and "hospital management support for patient safety" (33%). The attitudes of CRNAs concerning the overall “patient safety rating” were lower than those of physician anesthesiologists. However, the absolute difference between these averages was not statistically significant (p = .53). There were no statistically significant differences in the dimensions of “teamwork,” “staffing and workplace,” and “hospital management support for patient safety” between CRNAs and physician anesthesiologists. A thematic analysis of free-text comments resulted in three main themes: staffing shortages, continuous organizational learning, and leaders' lack of support for patient safety and communication openness. Based on the study results, the authors recommend that improvement efforts focus on improving staffing and leadership support for patient safety to create meaningful positive changes in the facility’s PSC.
