Managing Perioperative Glucose of Diabetic Patients

BACKGROUND: Perioperative glucose management is an essential component in the care of diabetic patients receiving surgery. Failure to assess and treat hyperglycemia in the perioperative setting leads to improper management of BG, increasing the risk of poor patient outcomes and increased costs from a prolonged length of stay. This project focused on understanding the barriers to glycemic control at the study site. METHODS: This QI project used a barriers survey as the primary tool for the paper. The sample population was the one hundred anesthesia providers at the selected Southern California hospital. The goal was to achieve at least a 20% response rate from the survey. Thematic analysis using NVivo was utilized to showcase the results further. INTERVENTION: Using a Qualtrics survey, anesthesia providers were asked to share their perceived barriers to glycemic control. Twenty-five out of the one hundred anesthesia providers completed the survey. Development of a CDS alert “pop-up” was also initiated. RESULTS: It was found that the two main barriers were access to supplies and personnel related. Providers also deemed the incorporation of an alert-based tool within the EMR as presumably useful. CONCLUSION: It can be concluded that access to supplies, continued education, and implementation of an alert-based tool can help increase the proper management of hyperglycemia in the perioperative period and help decrease adverse events from occurring in the postoperative period.
