Hospital-Acquired Pressure Injury Prevention Bundle in the Intensive Care Unit

Abstract:
Pressure injuries impact over 2.5 million adults in the United States each year, posing a significant risk to patients in the Intensive Care Unit (ICU) due to their underlying health conditions. Hospital-acquired pressure injuries (HAPIs) are prevalent, stemming from factors such as immobility, insufficient preventive measures, and poor nutrition, affecting 10% to 20% of critical care patients. In a trauma facility’s ICU, the HAPI rate was 11.6 per 1,000 patient days, contributing to healthcare costs of approximately $26.8 billion annually. This project was designed to enhance patient outcomes and reduce costs by updating the care bundle protocol with an effective prevention strategy that included accurate wound staging, appropriate HAPI reporting, and timely nutrition consultations. Convenience sampling was used to gather 26 ICU nurses in a 24-bed ICU within a 383-bed nonprofit hospital in Southern California. A pre-test using the adapted Pieper Zulkowski Pressure Ulcer Knowledge Test (PZ-PUKT) was administered, followed by a focused 10-minute educational session on HAPI staging, reporting, and nutrition. A post-test was conducted, supported by weekly surveillance data and chart audits. While the Chi-square Test yielded a composite p-value of 0.079, indicating no significant statistical correlation, there was a clear improvement in HAPI staging, reporting, and nutrition consultations in the 11 weeks following the education, resulting in a substantial reduction in the HAPI rate of 8 per 1,000 patient days. This reinforces the critical need for ongoing education to prevent injuries and highlights the potential for broader adoption of the care bundle across departments.
