Implementation of a Telehealth Patient Experience Survey: A Pilot Study

Abstract:
The Centers for Medicare and Medicaid Services mandate healthcare organizations to collect patient experience (PE) data for funding eligibility, yet cash-based practices (CBPs) are exempt, resulting in limited PE data collection. PE surveys are crucial for assessing patient perceptions and improving outcomes. Traditional surveys often focus on face-to-face interactions, with few addressing telehealth services. This Doctor of Nursing Practice (DNP) pilot study employed the Plan-Do-Study-Act (PDSA) framework to implement the Virtual Client Experience Survey (VCES) within a cash-based psychiatric practice. This validated survey utilized the Agency for Healthcare Quality's (AHRQ) six domains of quality—efficient, timely, effective, patient-centered, safe, and equity—alongside a free-text question on telehealth experiences. Convenience sampling recruited eligible patients over a six-week period, resulting in nine participants from the 52 who met inclusion criteria. Results of the VCES ranged from “agree” to “strongly agree,” with mean scores reflecting positive perceptions across domains (Efficient μ=3.64; Timely μ=3.29; Effective μ=3.38; Patient-centered μ=3.57; Safety μ=3.65). Despite recruitment challenges, the study demonstrated the viability of collecting PE surveys in CBPs, suggesting the potential for improving clinical outcomes, patient retention, and referrals. Recommendations include increasing survey reminders, ensuring sustainability, enhancing navigation of virtual platforms, reviewing privacy features like background blurring, offering flexible provider availability, and closely monitoring survey completion rates. The project underscores the importance of PE surveys in adapting to evolving healthcare delivery modalities and enhancing patient-centered care.
