Improving Intraoperative Glucose Management of Diabetic Patients

Abstract: |
Failure to test intraoperative blood glucose (BG) values deters dysglycemia identification, leads to inappropriate or absent treatment, and contributes to undesirable postoperative patient outcomes and increased hospital costs. Diabetic non-cardiac surgical patients, a known vulnerable population, require frequent monitoring of BG levels and optimal perioperative management to adhere to current best practice guidelines and reduce complications. This quality improvement project aimed to re-educate anesthesia practitioners by providing education on the Emory protocol and best intraoperative BG management at the proposed study site. Additionally, a CDS tool was implemented within the EMR to remind providers to check intraoperative BG. The implementation fidelity of this project’s proposed interventions was assessed by a thorough analysis of the four concepts incorporated within the definition of fidelity. The four concepts analyzed included adherence, exposure, participant responsiveness, and quality of the project’s interventions. By strengthening the practitioner's knowledge and preparedness to test and manage intraoperative BG, improved adherence to the glycemic management protocol and increased intraoperative testing were supported, thus improving patient care and outcomes.
