Exploring Anesthesia Practitioners’ Perception of Ambulatory Surgery Center Patient Safety Culture

Abstract:
Ambulatory surgery centers (ASCs) nationwide are performing more surgeries on more complex patients with an increasing demand for efficiency and cost reduction. Changes in technology, an aging patient population, increasing demand for surgeries, and changes in Medicaid and Medicare reimbursement are fueling this trend, and it shows no signs of stopping. Patient safety and research in this area have lacked prioritization compared to inpatient settings despite increasing safety concerns. This project performed a secondary data analysis of results from the 2023 Agency for Healthcare and Quality (AHRQ) Surveys on Patient Safety (SOPS) in ASCs and explored the current patient safety culture among anesthesia practitioners within participating ASCs. Post-hoc analysis compared patient safety responses between physician anesthesiologists and Certified Registered Nurse Anesthetists (CRNAs). A total of 190 surveys were completed by anesthesia practitioners (120 physician anesthesiologists and 70 CRNAs). Results were analyzed with descriptive, comparative, and correlational statistics. Physician anesthesiologists scored higher than CRNAs on every item and composite measure of patient safety, with many differences that were statistically significant. Of the AHRQ composite measures, staffing, work pressure, and pace had the lowest percent positive response rate (PPR) at 79.6%. Communication about patient information was the highest-scoring composite measure at 93.7%. Nearly all composite measures have decreased with each consecutive ASC SOPS, demonstrating a concerning trend. Best practice recommendations were formulated based on this secondary data analysis and a review of the current literature.
